In an effort to create a safe place for all to enjoy movies at the Poncan, we require all children 15 years of age or younger to be accompanied by someone who is at least 16 years old. Guardians may send their children to the free movies with another person who is 16 or older after submitting this form. The key tag received at your first event may be used for entry at future events.

Only one person from each group / family needs to fill out the form.

As the parent or legal guardian of the child(ren) named in this form, I hereby give my full consent and approval for my child to participate in an event at the Poncan Theatre.

I understand that there are risks in traveling and other related activities incidental to my child’s participation, and I am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating in the "free movie" events and that my child is healthy and has no physical or mental disabilities or conditions that would restrict full participation in these activities.

In addition to giving my full consent for my child’s participation, I do hereby waive, release and hold harmless The Poncan Theatre Company, Inc. , its officers, employees, sponsors, supervisors, and representatives for any injury that may be suffered by my child in the normal course of participation in the activities incidental thereto, whether the result of negligence or any other cause.

In the event that any damage to equipment or facilities occurs as a result of my or my family's willful actions, neglect or recklessness, I acknowledge and agree to be held liable for any and all costs associated with any actions of neglect or recklessness.
